1. The parent(s), family member, friend or town mayor should submit the following justifications and information, and file an application with the city (town) hall of the place where the household registration of the individual is located for the person to be transferred to our Institution.
(1) Full family household registration transcript shall be applied from the household registration office where the household registration of the individual is located.
(2) Property and income proof for the individual under application and his/her parents should be applied from the National Property Bureau or the tax authorities.
(3) One copy of the ID card of the individual and one copy of the booklet for the disabled (for individuals with multiple disabilities, it should be specified whether there is moderate intellectual disability).
(4) Specify name, address, telephone number of the contact person and his/her relationship with the individual.
(5) In case of low-income household, please attach proof.
2. After the city (town) hall accepts the application, the above-mentioned proof and information shall be submitted to the city or county government. The social service bureau of the city or county government shall review related information of the individual and determine the amount of monthly caring fee and governing subsidy as well as the amount to be paid by the individual’s family in accordance with the family and economic situation of each individual. The case is then transferred to our Institution for admission.
